A referral is required for all appointments.
Dr. Kunzer does not accept pain management referrals or provide suboxone medication management.
All patients must sign a medication informed consent form and a controlled substance agreement contract at the time of their appointment with Dr. Kunzer.
Legal guardians are required to sign for minors and guardian-appointed adults.
This clinic utilizes the Michigan Automated Prescription System (MAPS) and e-Prescribe (eRx) for all patient care.
