We are in network with Blue Cross Blue Shield and Medicare insurance plans. We are considered out of network with all other insurance plans and also offer self pay options.

For patients with out of network benefits, we can provide a Superbill documenting services provided, which the patient may submit to their insurance company for potential reimbursement according to the terms of their individual plan.

We recommend that referring providers encourage patients to contact their insurance carrier prior to scheduling to confirm their specific benefits, coverage requirements, authorization requirements, and out of network reimbursement policies.

Referral Documentation Checklist

Please provide the following information and documentation when referring a patient for evaluation.

□   Referring Provider Information

• Name of referring provider

• Provider phone and fax number

□   Patient Demographics

• Patient full name

• Date of birth

• Address

• Phone number

□   Reason for Referral

• Primary concerns, relevant symptoms, and diagnoses

• Requested treatment services: Neuropsychological evaluation, medication management, psychotherapy

□   Insurance Information

• Insurance company, member or ID number, and group number

• If the referral is related to an automobile accident or workers' compensation claim, please also provide: Claim number, name of insurance adjuster, and adjuster's phone number

□   Supporting Clinical Documentation

Please send any pertinent medical, clinical, or office notes that may assist with the evaluation, including a current medication list.
